Making my first Credit List ....help.....

hey gang,

Any one have an example of a good credit list? or any advice on what to do, or not to do when making one?

I also had a fairly specific question. If another company outsources some animation work to the company I'm working for, do I mention that other company, my company, or the owner of the show in my credit list?
