There are two separate ways to submit an entry form to the Ottawa 04 International Animation Festival:

1. Fill out the Online Entry Form.
2. Fill out the Downloadable Entry Form
   and mail or fax it to the address below.

Entry deadline is July 2, 2004. All submissions must include a VHS or DVD copy of the film along with a director's photograph, biography and 3 stills from the film. We cannot accept customs charges. Please do not label a value of over $15 CAN on your package.
